IMPORTANT INFORMATION REGARDING YOUR INSURANCE BENEFITS
Dear Prospective or Returning Patient,
Over the past decade, insurance company policies have dramatically affected the abiltiy to provide medically necessary physical therapy care. Unfortunately, no longer is the decision to provide appropriate medical care determined by the physican, physical therapist and patient.
Regardless of what your health care team may feel is necessary, even if ordered by your physician through a physical therapy prescription, your actual benefit is determined by your insurer. This determination may be through a pre-determined benefit plan or through a third party, hired by your insurer, who will make a medical decision about you based on paper work we must submit to them. This paper work is specific to each individual insurance company and must substantiate the need for ongoing physical therapy.
The Patient Registration Form that you will be filling out shortly will contain specific questionnaires regarding your condition. It is ESSENTIAL that you take the time to fill out these questionnaires appropriately, identifying your symptoms as accurately as possible so that we can ultimately receive the number of treatment sessions necessary to guide you to a complete recovery. Without appropriate documentation of your physical limitations your insurer will not authorize continued physical therapy.
